Each District 113A health office is staffed by a Registered Nurse. The nurses provide emergency care to students who are injured at school or during school-related activities.

The nurses also collaborate with parents to address the needs of chronically ill children so that the children can remain in school. Health office procedures and requirements are regulated by Illinois School Code and the Illinois Department of Public Health.

Reminder, all incoming sixth graders must have an updated physical with immunizations and dental examination on file with the school nurse before Oct. 15.

If your child joins a fall sport, sports physicals must be up to date before a student can try out or participate.

  • Physical, dental and eye examinations and immunizations, prescribed by State Law, are required of students in the categories below. Parents are requested to have their children examined by a licensed practitioner and to return physical, dental and eye examination forms and immunization records to school before the school year begins. All physical exams and immunizations must be filed by October 15 of the school year, or by district policy, the student will be excluded from school.
